To request routine unit maintenance for Country Squire Homeowners.

1. Download and fill out the form.

2. Email the completed form to the Country Squire email, countrysquire1314@gmail.com, return it to the clubhouse mailbox by the bulletin board, 1309 N. 43rd Street, or take it to the mailbox at Beth Downings at 4213 Country Lane.

3. Blank work order request forms are available at the clubhouse mailbox by the bulletin board.

4. Request will be reviewed at the next board meeting.
